This website is a part of the LTC: Let’s Take Care consumer awareness campaign for long term care in Milwaukee County.

This campaign is a component of Connecting Caring Communities (CCC), a collaborative project supported by the Robert Wood Johnson Foundation through its Community Partnerships for Older Adults (CPFOA) national initiative. CPFOA seeks to foster community partnerships that are improving long term care and supportive services systems to meet the current and future needs of older adults.

Milwaukee’s CCC project also receives generous support from three local foundations: the Greater Milwaukee Foundation, Helen Bader Foundation, and Faye McBeath Foundation.
